New Patients

Criteria

To register as an NHS patient at Community Health Centre you should fit the following criteria:

1. To be living permanently within the catchment area (See below Map of the catchment area and postcode checker).
2. To have United Kingdom Resident Status or equivalent documents.
3. To accept all the terms of the GP Patient Charter with regard to the responsibilities of patients.
4. To provide all required documentation to complete the registration process (see below).

To go ahead and register you must complete the following documents and bring them into reception with required supporting documents. Please see How To Register for further details.

2. Family Doctor Service Registration Form
